Privacy Statement
We collect personal information from you, including your name, contact information and services you access at the Centre.
We collect your personal information in order to: report to our funders, to support our funding applications, and for our annual report and our outcomes report.
All information used for reporting is anonymised. We won’t provide identifying information about individuals.
Besides our staff, we may share this information with:
- funders to support funding applications and accountability reports
- other health care professionals or government organisations if there are safety issues. This is to help keep people including yourself safe. If this happens we will tell you if we are doing this.
- Courts, Oranga Tamariki or Police and other government organisations as requested under relevant laws (for example Section 66 of the Children’s Act). In the event a request is received we will let you know what information is being asked for and how we will respond.
We keep your information safe by holding it in locked cabinets and digital files are password protected.
We keep your information for 10 years at which point we securely destroy it by shredding paper files and deleting digital copies.
You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
